AGENDA

9:45   - 10:00

Introduction

10:00   - 11:00

HCC Education for Inpatient and Outpatient CDI

In the evolution to value-based care, understanding HCC concepts is vital, particularly for those accustomed to acute care coding and clinical documentation improvement. The speaker will discuss how this coding system works by highlighting the interaction of weights and overriding categories. An emphasis on medical record documentation will include a review of the problem list, past medical history, and medication list as well as other appropriate clinical and administrative criteria that is often missed.

The speaker will define the different HCC models and discuss the utilization of both inpatient and outpatient records to reflect the patient’s health risk. Since determining a patient’s risk adjustment factor (RAF) is the intended outcome, the speaker will guide CDI specialists in identifying common missed query opportunities that reflect appropriate RAF scores for patients.

11:00  -  12:00 Deep Dive into Clinical & Coding Denials

Denials may or may not be a part of your CDI department’s functions in a formal setting, but every CDI program can play a part in denials prevention. By understanding the diagnoses that are frequently seen in hospital denials and appeals departments around the country, the learners will be able to identify high-risk records and anticipate an insurance company’s denials rationale. CDI professionals will be taught strategies to engage providers in the query process to help make diagnoses as “audit-proof” as clinically possible.

 12:00  -  1:00  Break

Coding Clinic for CDI- 2020 in Review

Published quarterly, coding clinics bring the newest official coding information and guidance to CDI and coders to help ensure medical records are coded to the fullest degree of specificity and integrity. This information provides CDI and coders examples by applying real-life scenarios that help with coding accuracy that align with the ICD-10- CM/PCS official coding guidelines. While COVID took the front seat in most coding and CDI programs, many non-COVID-related coding clinic updates were often overlooked. In this presentation, the speaker will provide you with the updates from 2020 that impact or influence CDI practice.

 2:45  -  4:15

Query Workshop

While understanding clinical and coding concepts are important factors in an effective CDI program, no single job function is as impactful as the query writing process. Anyone can read a record but not everyone can take the clinical indicators and the context of the patient’s illness and translate that into missed documentation opportunities. In this workshop, the speaker will review compliant query practice utilizing the AHIMA/ACDIS Guidelines for Achieving a Compliant Query Practice and work with the learners to apply the standards within the brief in clinical case studies. During our breakout sessions, groups will work to write compliant queries based on the clinical scenarios and critique written queries for opportunities to achieve best practice.
